People's Open Access Educational Initiative, Manchester, United Kingdom

A trained workforce of health professionals is essential, especially for low to middle income countries (LMIC), which have to cope with a wide range of health problems. Local universities are being vastly oversubscribed for face to face courses and fees for overseas universities, including e-learning distance programmes, are higher than can be afforded by most potential students in these countries.

Peoples-uni is helping to build Public Health capacity using Internet-based e-learning, which has the exciting potential to deliver high quality learning resources anytime and anywhere, and although Internet access is by no means universal it is improving quickly.

April, 2008:
Africa Centre for HIV/AIDS Management, Matieland, South Africa

The scenario: Sub-Sahara Africa houses a staggering 65% of people living with HIV and AIDS globally. This is a disproportionate burden, seeing that the subcontinent only has 10% of the world’s people and 1% of the resources. In South Africa alone around 5,5 million people are HIV positive. The disease primarily kills people in their most productive years and exceeds any other threat to the wellbeing of employees. The economic impact is therefore substantial.

Taking action: Towards the end of 2000 SA’s presidency challenged Stellenbosch University to develop a programme that takes HIV training and capacity building to the managers and labour leaders of the workforce. The rationale behind the challenge was that, regardless of what is done in the community, prevention and care had to be extended to the workplace in order to make a dent in the epidemic. This led to the development of the Postgraduate Diploma in HIV/AIDS Management (PDM), first presented in 2001. Today the centre also presents a masters programme.

Moi University Kenya: School of Public Health

Brief History: The School of Public Health (SPH) was established in October 1998 by the Moi University Council in recognition of the need for training high-level manpower in public health. The School's training programmes have been developed to meet Moi University's mission of producing skilled and practical health professionals who contribute to the realization of the country's national goals. The School thus seeks to provide high quality interdisciplinary training in public health for a range of health personnel, including policy makers, medical practitioners, academicians and researchers.

Purpose of the School: The School of Public Health is a postgraduate research-oriented training institution, which also offers professional public health services to the government, non-governmental agencies and communities. Whereas many countries in Africa, including Uganda, Tanzania, Ethiopia and Ghana have established institutes or schools of public health to address their manpower needs in the areas of public health, in Kenya, such training has largely been attainable in European and North American Universities at a very high cost. The Moi University's School of Public Health is currently the only purposely established institution in the country that offers opportunities for local training of public health professionals at postgraduate level, which is affordable, context specific and responsive to the country's needs

Mission and Vision: The mission of the School of Public Health is to promote improvements of health of the people of Kenya. The vision of the School is to become a regional and international centre for excellence in the training, research, practice and consultancy in public health.

June, 2006:
University of Debrecen, Ungary: School of Public Health

The University of Debrecen, like other integrated institutions of higher education in the country, was formed on January 1, 2000 by (re)uniting formerly independent institutions. Its historical roots reach back to the foundation of the Reformed College of Debrecen (1538), whose three academic sections later served as the base for the Hungarian Royal University of Sciences, created by Statute no. 36 in 1912. With this past of more than 450 years, the University of Debrecen is the oldest institution of higher education in continuous operation in Hungary based in the same city. Higher education in agriculture began in 1868, when the National Higher School of Agriculture was formed in Debrecen.

Solving regional, national and international problems of any kind requires broad-based efforts of many sectors relying on a wide range of disciplines. This is even more true for combatting health problems of populations the roots of which are intertwined and wide-ranging. Public health aims at the improvement of health, prolongation of life and improvement of the quality of life in populations by using a multidisciplinary theoretical approach translated into effective intersectorial practice. The Hungarian Government, recognising that these elements of effective public health can be implemented only by highly trained professionals, established the School of Public Health as an independent institute within the University of Debrecen financed in part by a World Bank loan. Postgraduate M.Sc. training in public health was launched in the School in September 1996 followed by launching an M.Sc. course in Environmental Health one year later. A Ph.D. programme in Preventive Medicine and Public Health started in September 1999, and M.Sc. courses in Quality Assurance and Improvement in Health Care, Health Promotion, Epidemiology are offered from September 2000.

The teaching staff of the school has been recruited on a national basis: distinguished lecturers of other universities as well as leading experts from national institutes of public health participate in the programmes in order to ensure the highest quality of postgraduate training. Accredited teaching laboratories of the School operate in four county offices of the National Public Health Service.

The School is a member of the Association of the Schools of Public Health in the European Region (ASPHER).

University of Hong Kong: Department of Community Medicine

In response to the SARS crisis and the recommendations of the SARS Expert Committee the university established the School of Public Health and the Public Health Research Centre inside of the Department of Community Medicine in 2004. The Master of Public Health (MPH) degree programmes were also introduced to address the continuing need for professional training in the public health sciences.

In the past 10 years the Department of Community Medicine has contributed significantly to the international literature and to health in the local population as highlighted below:
The department has a long history of tobacco control, advocacy and related research achieving international recognition. In 2005, Dr SM McGhee, Dr SY Ho, Dr M Schooling, et al, with Professor R Peto of Oxford University and Dr KH Mak of Department of Health were the first world-wide to report a dose-response relationship in the risk of death from stroke after exposure to second hand smoke and provided critical support for legislation on smoke-free policies in workplaces. (published in British Medical Journal).
The department has also a long history of research in environmental pollutants. In 2002 it was the first to demonstrate worldwide the independent effects of air pollutants on deaths from cardiopulmonary disease and benefits of interventions reported in the Lancet. It also reported the first study on dioxins and PCBs in breast milk in Hong Kong, jointly with Departments of Paediatrics and Community Medicine in Chinese University of Hong Kong, in a World Health Organization collaborative study.

Hong Kong has made remarkable gains in health, exemplified very low infant and maternal mortality, increase in life expectancy, and the elimination of many communicable diseases through the application of sound public health principles and practices. Although much has been achieved, an ageing population, growing prevalence of chronic diseases (cancer, heart disease), infectious diseases, smoking, changes in lifestyle and behaviour continue to present challenges to the public's health in Hong Kong in the 21st century.

Johannesburg: National Institute for Occupational Health

The National Institute for Occupational Health is South Africa’s major centre for occupational health development, training, service support and research, with a staff of 90 people of whom half are scientists.

The National Institute for Occupational Health (NIOH) has existed in various forms since the 1950s. The South African Institute for Medical Research (SAIMR) was founded to study diseases in mine workers, particularly pneumoconiosis and tuberculosis. The delayed onset of these occupational diseases was recognised in relation to compensation and South Africa introduced post mortem examination of cardiorespiratory organs of all deceased miners and ex-miners. The need for expertise in occupational pulmonary pathology led to a pneumoconiosis unit being established within SAIMR to do both postmortems and research. This major statutory function of the NIOH, namely post mortem examination of the cardiorespiratory organs of all deceased miners and ex-miners for compensation and research purposes, continues half a century later to provide unique opportunities for advancement of global knowledge through service-based research, publication and training.
